　　1. 深圳市国际招标有限公司（以下简称“招标机构”）受招标人委托，邀请合格投标人就惠州市中心人民医院的下列货物和有关服务提交密封投标：

　　2. 有意向的合格投标人可在深圳市国际招标有限公司惠州经营部得到进一步的信息和查阅招标文件。

　　3. 投标人资格：

　　(1)投标人必须是合法运作的独立机构（提供营业执照副本复印件加盖公司公章或商务执业许可证）。

　　(2)投标人必须为必联网（http://www.ebnew.com）或机电产品招标投标电子交易平台（http://）注册用户。

　　(3)投标人必须是所投产品的制造商或制造商有效授权的合法代理商，并提供设备制造商专项授权书或代理证书复印件（投标人是设备制造商的除外），并加盖投标人公章，且在招标机构登记并购买了招标文件。

　　(4)投标人不得与买方或招标机构有利害关系。

　　(5)两家以上投标人的投标产品为同一家制造商或集成商生产的，按一家投标人计算。

　　(6)国内投标人需具有《中华人民共和国医疗器械生产许可证》或《中华人民共和国医疗器械经营企业许可证》（提供证书复印件并加盖投标法人公章）。

　　(7)投标人所投设备必须具有《中华人民共和国医疗器械注册证》（提供证书复印件并加盖投标人公章）。

　　(8)法定代表人证明书或法定代表人授权书。

　　4.有意向的投标人可从即日起至2020年1月31日17时00分止(节假日除外)，每天上午9：00～11：30、下午2：30～5：00 (北京时间) 在惠州市花边北路3号宏城商务大厦5层506室 (深圳市国际招标有限公司惠州经营部)现场购买招标文件（请购买人携带提供营业执照副本复印件加盖公司公章或商务执业许可证复印件加盖公司（原件备查）、法定代表人证明书原件、法人代表授权书（购买招标文件）原件、提供所投产品中华人民共和国医疗器械注册复印件加盖公章），本招标文件每包售价人民币800元或130美元，售后不退。

　　5. 所有投标书应于2020年2月19日10：00(北京时间)之前递交到深圳市罗湖区嘉宾路深华商业大厦裙楼六楼633室(深圳市国际招标有限公司)。

　　1. Shenzhen international tendering co., LTD. (hereinafter referred to as \"bidding agency\") entrusted by the tenderer, qualified bidders are invited to center of Huizhou central people's hospital  of the following goods and related services to submit sealed bids:

　　2. Qualified bidders of interest may in shenzhen international tendering co., LTD., huizhou sales department to get further information from and inspect the bidding documents. 

　　3. The bidder qualifications: 

　　( 1 ) the bidder must be an independent institution that operates legally ( provide a copy of the business license, affix the company's official seal or business practice license ).

　　( 2 ) Bidders must be registered users of BIDLINK (http://www.ebnew.com) or the Electronic Trading Platform for Tendering and Bidding of Mechanical and Electrical Products (http://).

　　( 3 ) the bidder must be the manufacturer of the invested product or the legal agent effectively authorized by the manufacturer, and provide a copy of the special authorization or agency certificate of the equipment manufacturer ( except that the bidder is the equipment manufacturer ), affix the official seal of the bidder, and register with the tendering agency and purchase the bidding documents.

　　( 4 ) the bidder shall not have any interest with the buyer or the tendering agency.

　　( 5 ) if the bidding products of more than two bidders are produced by the same manufacturer or integrator, it shall be calculated by one bidder.

　　( 6 ) domestic bidders shall have the medical device production license of the people's Republic of China or the medical device business enterprise license of the people's Republic of China ( provide a copy of the certificate and affix the official seal of the bid legal person ).

　　( 7 ) the equipment invested by the bidder must have the medical device registration certificate of the people's Republic of China ( provide a copy of the certificate and affix the official seal of the bidder ).

　　4. Interested bidders can purchase bidding documents from now on to 17: 00 on January 31, 2020 ( except holidays ), at room 506, floor 5, hongcheng business building, No.3, huabian north road, Huizhou city ( Huizhou business department, Shenzhen international tendering co., ltd. ) every morning from 9: 00 to 11: 30 and from 2: 30 to 5: 00 ( Beijing time ) ( please bring the copy of business license with the official seal of the company or the copy of business practice license with the company ( original ), the original certificate of legal representative, the original letter of attorney of legal representative ( purchase bidding documents ), and provide the medical devices of the people's Republic of China.The bidding documents are priced at RMB 800 yuan or US$ 130 per package and will not be returned after sale.
